Current treatments for acute ischemic stroke (AIS) have limitations, including a narrow therapeutic window and risks of intracerebral hemorrhage, leaving a significant portion of patients without effective options for restoring blood flow to the brain. Device-based endovascular thrombectomy is limited by its ability to retrieve clots only in large vessels and is unavailable in many acute care centers. The majority of AIS patients do not receive acute treatment, leading to increased mortality and disability.

Basking Biosciences is developing BB-031, a reversible RNA aptamer designed to target von Willebrand Factor (vWF), a key driver of blood clotting, for the treatment of acute ischemic stroke. BB-031 is engineered to provide a rapid-onset, short-acting solution for reopening blocked arteries and is designed to be quickly neutralized in the event of bleeding complications using BB-025. This approach aims to expand the eligibility of the AIS patient population for acute revascularization by offering a safer and more effective therapy that can restore blood flow beyond the therapeutic time window of standard-of-care treatments. BB-031 is currently being evaluated in a Phase II clinical trial.
